Chronic Disease Management

For patients with long term medical conditions (chronic diseases) it is most important to have a regular General Practitioner as the co-ordinator of medical care. Hawker Medical Practice is active in managing patient with long term medical conditions such as diabetes, asthma, organ failure etc. Besides managing general medical needs, we are able to initiate specific care planning in managing Chronic Diseases which are funded by Medicare. Please speak to your doctor about it.

Enhance Primary Care (EPC) Care plans are extremely helpful for people with chronic diseases. The plans are prepared using a collaborative approach involving Doctors and patients, Practice Nurses and Allied Health Professionals. The care plans are documents that summarise the important areas of health concern that have been agreed between the patient and the caring team. They outline a plan of management, the actions needed to be taken by the patient and the team members involved in assisting in the patient's recovery. 

EPC Care plans are bulk billed (one plan allowed every two years) and renewed on a regular basis. Once a care plan is in place it allows the patient to access a Medicare rebate for services provided by the allied health professionals who are involved in the care plan.
